New Patient Info

    At  Lotus Acupuncture, our goal is  to provide you the best care and treatments.  Please complete the
new patient paperwork below and bring any relevant lab work to the initial appointment

          

We recommend the following to prepare for the first acupuncture appointment:
   Eat at least 2 hours prior to treatment
   Avoid heavy meals, fasting or long periods prior to appointment             
  Wear loose fitting clothes
  Use restroom prior to arrival 
   Try not to brush tongue day of appointment 

Insurance

Our clinic participates with some insurance companies.  However, insurance plans vary in coverage
and acupuncture may not be included. Please contact your insurance to verify that Lotus Acupuncture
is in/out network.​

We do not bill directly to insurance company but will provide a superbill receipt for 
to submit to your insurance company for reimbursement.

We welcome any HSA or FSA card.
